District Health & Family Welfare Samiti, Jalpaiguri is going to organize Walk-In-Interviews for the following posts under NHM/NUHM/XVF Health Grant Programmes as per schedule given below
Name of the Post--Ophthalmic Assistant
Essential Criteria: -
1 . Passed Higher Secondary with Physic , Chemistry & Biology .
2 . Must have 02 (Two) Year Diploma in Paramedical Oph thalmic Assistan t Cou rse OR 02 (Two ) Year Di ploma co urse in Optometry & Ophthalmic technique recognized by the Gov! . of Wes t Bengal
Name of the Post--Staff Nurse
Essential Criteria: Completed GNM Course - from an institute recognized by Indian Nursing Council.
Name of the Post-ANM/CHA--Must have passed ANM Course from an institute recognized by Indian Nursing Council and be registered with the West Bengal Nursing Council
Should be proficient in Bengali language

In case OBC candidates Category 'A' or 'B' must be mentioned specifically in the caste certificate otherwise the candidate will be treated in 'Unreserved Category'. Original & Self attested Photocopy of Experience Certificate (Experience certificates must consist of Name of the post, Employee's Name, Date of Joining (DOJ) and Date of Leaving (DOL) otherwise experience certificates will be treated as invalid). Age relaxation for SCISTIOBC-(A&B) candidates as per Govt. norms. Original & attested Photocopy of Photo copy of Voter card IAadhaar card I other address proof. Proof regarding permanent residential status (Voter Card/Ration Card) should be duly attested by a Gazette Officer or 'Group-A' Officer of the State Government. Self attested recent 2(Two) copies passport size photo to be pasted one in application form another copy of passport size photograph at the time of interview Self attested photocopy of all required documents as per post criteria. Registration will be started at above mentioned venue from 10:30 AM on scheduled date. The candidates who will appear after 12:00 Noon should not be allowed for registration. Any omission/suppression of information shall lead to rejection of application or candidature at any stage of the process without further intimation. The conditions so prescribed shall not be relaxed
